Normal Isn’t the Same as Optimal: How to Read Your Fertility Labs
If you’ve ever been handed a lab report with the word “normal” next to every line and still felt like something was off, you’re not imagining it. Normal and optimal are two different things, and the gap between them is where a lot of fertility answers live.
Here’s the short version. A reference range tells you whether your result is unusual for the general population. It does not tell you whether it’s ideal for getting and staying pregnant. Plenty of people sit comfortably inside “normal” while a few of their numbers are quietly less than ideal for conception.
Why does “normal” not mean “optimal”?
Because reference ranges are built to catch disease, not to fine-tune fertility. They’re drawn from a broad population, so the bar is set wide. A result has to be fairly far off before it gets flagged.
That’s the right tool for spotting a clear problem. It’s the wrong tool for the question you’re actually asking, which is whether your body is in the best possible place to conceive. Two people can both be “normal” and be in very different spots for fertility.
Which labs are worth a closer look?
These are common places where “in range” and “ideal for fertility” can part ways. None of this is a diagnosis, and none of it is a reason to panic. It’s a list of things worth talking through with your team:
- Thyroid (TSH and beyond). A TSH can be inside the lab range but higher than many fertility-focused providers like to see when you’re trying to conceive.
- Blood sugar and insulin. Markers can read normal while insulin resistance is quietly affecting ovulation and egg quality.
- Vitamin D. Often “normal” but on the low end, when a more comfortable level may support the process.
- Iron and ferritin. Low-normal stores can affect how you feel and function.
- Other thyroid antibodies and inflammation markers, which a basic panel may not include at all.
The point isn’t that any one of these is the answer. It’s that “normal” stopped the conversation when it should have started one.
How do I know what my numbers should be?
You bring the question to someone who’s looking at fertility specifically, not just screening for disease. The same number can be read very differently depending on the goal.

I think of a couple who came to me using reciprocal IVF, where one partner provided the eggs and the other carried the pregnancy. The partner carrying had been set up for transfer with almost no attention to her own health, because she wasn’t the one doing the retrieval. That single assumption left a lot on the table.
When we actually looked, there was plenty to work with. Her thyroid was suboptimal, her fasting insulin was high, her vitamin D was low, and she was barely drinking any water. She had a clear PCOS picture, what’s increasingly being called PMOS, that no one had done anything with, because in this pregnancy she was treated as just the carrier. Her own history hinted that something had been going on for a long time, and no one had looked into it.
She wasn’t thrilled with my recommendations, and I understand why. They asked something of her. But she followed them to the letter, and the change in her numbers was real. They’re pregnant now, and I’m certain it’s because of the work she did to get her own health ready before transfer. Carrying a pregnancy is its own physiology, and it deserves the same optimization we give the eggs and the sperm.
What should I do with a “normal” report?
Don’t treat it as a closed door. Ask three questions: Is each result optimal for fertility, or just within range? Is anything low-normal or high-normal that’s worth nudging? And what wasn’t tested at all? (More on that in the tests you probably haven’t had.)
Normal is a fine place to start. It’s a frustrating place to stop.
